Members of the team will be responsible for medication management quality, cost, utilization and clinical stewardship activities for populations managed and at a program level.

MSHS seeks a Clinical Pharmacy Manager that reports to the Executive Director of Pharmacy, Ambulatory Care and Population Health. The Clinical Pharmacy Manager will have direct patient care responsibilities aligned with care delivery goals as dictated jointly by Pharmacy and health system leadership. The Pharmacy Manager will have direct patient responsibilities including both in-person and centralized management activities. Manager may develop collaborative drug practice agreements with providers (MDs) at practices. Managers will lead medication management performance improvement initiatives. Managers are expected to manage pharmacy students and residents as requested. Manager will assist with design, implementation and evaluation of all related pharmacy initiatives.

Role Summary

Responsibility #1 Delivery of clinical pharmacy services with a focus on chronic disease management

  • Promote clinically rational drug therapy and sound pharmaceutical care through the development of new (or expansion of existing) pharmacy practice programs, drug therapy policies, and other programs
  • Serve as a clinical pharmacy resource for the health-system and its affiliated pharmacy services for the CIN
  • Work collaboratively with hospital-based clinical staff to operate the Medication Assistance Program (MAP)

Responsibility #2 Pharmacy Strategy and Innovation

  • Identification, development, and implementation of new pharmacy strategies and innovation pilot projects across responsible clinics.
  • Provide education, training and work direction to clinic staff and practitioners as required.

Responsibility #3 Precepting

  • Serve as a preceptor for the PGY-2 residency program and students from local colleges of pharmacy
  • Provide scholarly activities and presentations at local and national meetings to advocate for and raise awareness of population health pharmacy roles and responsibilities

Responsibility #4 Management

  • Develop and scale condition and medication management program, manage goals of pharmacy integration across MSHS, CIN employed or voluntary practices.
  • Support financial, quality and utilization analytics and performance improvement evaluations.
  • Serve as pharmacy representative on behalf of MSHP as required on organizational committees.

Education and Experience

  • Graduate of an ACPE-accredited School of Pharmacy with PharmD
  • Completion of PGY1 Pharmacy Residency (Pharmacy or Community-Based Pharmacy) required
  • Completion of PGY2 Pharmacy Residency (Ambulatory Care) preferred
  • Three years health system experience with clinical training; ambulatory practice setting preferred
  • Must be certified (or eligible for certification within 2 months of hire) to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department
  • Immunization certification and current BLS certification preferred
  • Must be board certified (BCACP) or obtain BCACP within 2 years of hire
  • NY State Pharmacy Licensure within 6 months of hire

Additional Skills and Qualities

  • Chronic disease management, care management
  • MTM experience, CDTM experience, pop-health experience
