Responsibilities
The Primary Care Physician will report to the Medical Director and responsibilities will include, but are not limited to:
- Provide comprehensive primary care to a panel of patients
- Collaborate with members of the medical team on chronic illness management pathways
- Assist with Performance Improvement work to provide comprehensive care that reduces unnecessary hospitalization
- Requirement in the Adult Medicine clinic and will treat adult patients only
Qualifications
- Experience with treatment of adult patients preferred
- Board Certification or Eligibility in either Family Medicine or General Internal Medicine
- Possess or be eligible for a New York State License
- Proof of full COVID-19 vaccination is required prior to hire, plus proof of subsequent booster vaccines within one month of eligibility
